How to file an application
Please complete the
Application for Disability Benefits Under the Québec Pension Plan and send us
without waiting for the Medical Report.
We can deem you to be disabled for a maximum of 12 months before the date on which we received your application. The same applies even if you have been disabled for a longer time.
You can file an application for a
pension for a disabled contributor's child at the same time as for disability benefits. To do so, you must complete the Application for a pension for a disabled contributor's child section on the same form.
Here are the steps to follow to apply for disability benefits under the Québec Pension Plan:
- Complete the Application for Disability Benefits – Québec Pension Plan portion of the form.
- Complete and sign the Consent Regarding the Release of Medical, Psychosocial and Administrative Information portion of the form.

Sending a document online service to send us your completed forms and other documents quickly. Your application will be processed faster, and the postal delay will be eliminated.
If you cannot use the online service, please mail us the first two parts of the form, that is, the Application for Disability Benefits Under the Québec Pension Plan and the Consent Regarding the Release of Medical, Psychosocial and Administrative Information at the following address:

Medical Report
Complete section 1 of the Medical Report. You must have the rest of the Medical Report form completed by your physician. Ask your physician to return it to us quickly. You will have to pay the fees charged by your physician for preparing the Medical Report.
You have a disability insurance with a private insurance company
You might already be receiving disability benefits from a private insurance company. The provisions could be integrated with the disability pension under the Québec Pension Plan (QPP). In other words, depending on the date on which we deem you to be disabled, your insurance company could
reduce or cancel your benefits.
You could also have to repay the benefits you already received, in whole or in part. In that case, you can ask us to make the repayment for you. To do so, complete the Consent and Application for Remittance of Disability Benefits to an Insurer form. A person representing your insurance company must complete it with you. The repayment and related interest are also taxable. Therefore, they are added to your income of the corresponding years.

Are you between ages 60 and 65?
Once we receive your application for a disability pension, you will automatically start receiving your retirement pension under the QPP. We suggest that you continue receiving your retirement pension until the decision to accept or refuse your disability benefit is rendered. If your application for a disability pension is accepted, you will receive a disability pension that will be combined with your retirement pension. We pay both pensions in a single payment each month.
If your application is rejected, you will only receive your retirement pension. You will then have two months to cancel payment of your retirement pension if you would like to do so.

You can also obtain the Application for Disability Benefits under the Québec Pension Plan form, including the Medical Report:
- at one of Retraite Québec's client services centres;
- from Services Québec;
- at your
Centre local de services communautaires (CLSC);
- at the office of your member of the National Assembly.
