Application for the Supplement for Handicapped Children Requiring Exceptional Care

You are entitled to this financial assistance if:

• you receive Family Allowance for the child concerned by the application for the Supplement for Handicapped Children Requiring Exceptional Care.
• you receive the Supplement for Handicapped Children for this child.
• the child is under age 18 and has severe and multiple disabilities, and/or his or her medical condition requires complex medical care. The child's condition must meet the eligibility requirements for a period expected to last at least one year.

If you receive Family Allowance but you do not receive the Supplement for Handicapped Children, you can send us your Application for the Supplement for Handicapped Children and your Application for the Supplement for Handicapped Children Requiring Exceptional Care at the same time.

If you do not receive Family Allowance or the Supplement for Handicapped Children, you can send your Application for Family Allowance Payments, your Application for the Supplement for Handicapped Children and your Application for the Supplement for Handicapped Children Requiring Exceptional Care at the same time.

The form the Application for the Supplement for Handicapped Children Requiring Exceptional Care includes:

• the application for the Supplement for Handicapped Children Requiring Exceptional Care;
• the Consent Regarding the Release of Medical, Psychosocial and Education-Related Information.

If you are filing an Application for the Supplement for Handicapped Children Requiring Exceptional Care at the same time as an Application for the Supplement for Handicapped Children for the same child, the information provided on the Application for the Supplement for Handicapped Children and all the documents you enclose will be consulted by the health professionals who are studying your Application for the Supplement for Handicapped Children Requiring Exceptional Care.
