Forms concerning Family Allowance

This form allows Retraite Québec to obtain, from the professionals involved with the child, the information necessary to assess the child's eligibility for Supplement for Handicapped Children Requiring Exceptional Care.

You do not need to send this form to Retraite Québec with your application for the Supplement for Handicapped Children Requiring Exceptional Care. If necessary, Retraite Québec will ask for the form, only after we have studied your child's file.
