You can send forms, documents as proof, medical file or other documents required by Retraite Québec to complete the study of your application related to:

  • the Québec Pension Plan;
  • the Family Allowance;
  • public-sector pension plans (RREGOP, PPMP, etc.) (members and beneficiaries);
  • a social security agreement (certificate of coverage only);

Use the SPP Portal or contact us by telephone to send us a document concerning:

  • pension plans in the private, municipal and university sectors;
  • voluntary retirement savings plans (VRSPs);
  • locked-in retirement accounts (LIRAs);
  • life income funds (LIFs).

Before sending your documents

  1. Choose one of the three following ways to do so:
    • Upload a document from your computer, your tablet or mobile phone.
    • Scan a document.
    • Take a photo of a document.
  2. Save your document in JPG or PDF format. Each document can be up to 7 MB maximum.
  3. Depending on your situation, follow our advice below and make sure your documents meet all the requirements.

If you would like to complete a PDF form onscreen, you can consult the steps to do so and add an electronic signature, if applicable. Two types of electronic signatures are accepted for certain forms.

  • Examine each picture before sending it to us (make sure that the pictures are bright, that the text is legible, that there are no shadows, that each document is complete, etc.).
  • You must be able to read the text on your pictures if you zoom in on your touchscreen. You can also send yourself the pictures by email and open them on a computer or laptop to make sure the text is legible.

Verify the quality of the originals received from health professionals

Your documents must include the headings, the bottoms of the pages and the signature of each health professional concerned. The writing must be clear even if it is in handwriting and the document must not be too dark (the brightness must be sufficient). Our medical team must be able to read all your documents in order to analyze your application.

If you must take pictures of a medical file, you can enter the page numbers manually if it is voluminous and does not contain any page numbers. If there are several documents, make sure not to mix them up. Do not forget to take pictures of both sides of the documents, if applicable.

What is another way to send your documents?

You can use the online form from the non-personalized services if you have a Social Insurance Number. If you are sending a document for another person, you must complete the online form. You will need the person's Social Insurance Number.
